In India, GLOBOCAN 2022 estimated approximately 81,748 new lung cancer cases, making it an important and growing healthcare challenge.

But Lung Cancer Treatment has changed dramatically. Today, patients may benefit from early detection with low-dose CT, advanced imaging, and bronchoscopy; minimally invasive Robotic or Uniportal VATS surgery; precision radiation therapy; chemotherapy; immunotherapy; and molecularly targeted medicines.

The key is not choosing a single treatment. Modern lung cancer care is about identifying the right combination and sequence of treatments for each patient.

Why is Early Detection of Lung Cancer so Important?

The stage at which lung cancer is diagnosed has a major influence on treatment and the possibility of cure. When non-small-cell lung cancer (NSCLC) is detected while still confined to the lung, surgery can potentially be curative in appropriately selected patients.

Unfortunately, lung cancer may produce few symptoms during its early stages. Screening selected high-risk patients with a low-dose CT scan (LDCT) can identify lung cancers before symptoms develop. Major international studies have demonstrated that LDCT screening in appropriately selected high-risk populations reduces lung-cancer mortality.

Screening is not recommended indiscriminately for everyone. Age, smoking exposure, general health, and other risk factors need to be considered before deciding whether screening is appropriate.

What are the Warning Signs of Lung Cancer?

* Persistent cough or a change in a long-standing cough

* Blood in sputum

* Unexplained breathlessness

* Persistent chest pain

* Recurrent pneumonia or chest infections

* Hoarseness of voice

* Unexplained weight loss or reduced appetite

* Persistent fatigue

These symptoms do not necessarily mean cancer, but persistent symptoms should not simply be attributed to smoking, pollution or infection without appropriate evaluation. This is particularly relevant in India, where symptoms of lung cancer may sometimes overlap with or initially be attributed to respiratory infections or tuberculosis.

Can Non-Smokers Develop Lung Cancer?

Yes. Smoking, including cigarette and bidi smoking, remains the most important preventable risk factor for lung cancer. However, lung cancer can also occur in people who have never smoked.

Second-hand smoke, air pollution, occupational exposures, previous radiation exposure and genetic susceptibility may contribute to risk. Importantly, some lung cancers in non-smokers contain specific molecular abnormalities that can be treated with highly effective targeted medicines.

Therefore, a non-smoker with a suspicious lung lesion still requires proper evaluation.

How is Lung Cancer Diagnosed?

Finding a lung mass on a CT scan is only the first step in the evaluation. Before deciding treatment, the multidisciplinary team need to determine the cancer type, exact stage, lymph-node involvement, whether it has spread elsewhere, whether the tumour has an actionable molecular alteration, and whether the patient is fit for surgery.

* Contrast-enhanced CT scan and PET-CT

* MRI brain when indicated

* Bronchoscopy and/or EBUS-guided lymph-node biopsy

* CT-guided lung biopsy when appropriate

* Pulmonary-function testing

* Histopathology and immunohistochemistry

* Molecular/genomic testing and PD-L1 testing when indicated

Accurate mediastinal lymph-node staging is particularly important because lymph-node involvement can completely change the treatment strategy.

When is Surgery Used for Lung Cancer?

Surgery remains one of the most important potentially curative treatments for early-stage and selected locally advanced NSCLC.

The objective is not simply to remove the visible tumour. Good oncological surgery requires complete removal of the cancer, adequate margins and appropriate lymph-node dissection or staging.

* Anatomical segmentectomy

* Lobectomy

* Sleeve or other complex lung-preserving resections in selected cases

* Pneumonectomy in carefully selected situations

Whenever oncologically appropriate, preserving healthy functioning lung tissue is an important consideration.

Robotic Lung Cancer Surgery: Precision Through Small Incisions

Robotic-assisted thoracic surgery (RATS) has become an important minimally invasive approach for lung cancer surgery. Instead of operating through a large thoracotomy incision, robotic lung surgery is performed through small ports.

* High-definition, magnified 3D vision

* Articulated instruments with excellent range of motion

* Precise dissection around pulmonary vessels and bronchi

* Improved access to confined areas of the chest

* Precise mediastinal lymph-node dissection

For appropriately selected patients, minimally invasive surgery can result in less surgical trauma, reduced postoperative pain, earlier mobilisation and faster functional recovery compared with traditional open thoracotomy.

Robotic surgery is a surgical approach – not a substitute for sound cancer surgery. Complete cancer removal and appropriate lymph-node dissection remain the primary objectives.

Uniportal VATS: Major Lung Surgery Through a Single Small Access Incision

Uniportal Video-Assisted Thoracoscopic Surgery (Uniportal VATS) allows selected major lung operations to be performed through a single small intercostal access incision. Procedures such as lobectomy, segmentectomy and selected complex thoracic operations can be performed using this approach in experienced hands.

* Smaller surgical access incision

* Reduced chest-wall trauma

* Potentially less postoperative pain

* Earlier mobilisation

* Faster functional recovery and return to normal activity

Current evidence supports good oncological outcomes with minimally invasive VATS approaches when the principles of complete tumour resection and appropriate lymph-node assessment are maintained.

The decision between Robotic surgery, Uniportal VATS, conventional VATS and open surgery should be individualised according to tumour anatomy, stage, previous treatment, patient fitness and surgical expertise.

Lung Cancer Treatment is no Longer ‘Surgery vs Chemotherapy’

Modern lung cancer treatment is increasingly multimodal. For some resectable cancers, treatment may begin with chemotherapy combined with immunotherapy, followed by surgery. Other patients may undergo surgery first and then receive chemotherapy, immunotherapy or targeted therapy depending on the final pathological stage, lymph-node involvement, molecular alterations, PD-L1 expression and overall recurrence risk.

Modern clinical trials have demonstrated that carefully selected patients with resectable NSCLC can benefit from integrating systemic treatment with surgery.

The key question has shifted from simply asking, “Can this lung cancer be operated on?”

It is: “What combination and sequence of treatments gives this patient the best opportunity for cure?”

Precision Medicine: Every Lung Cancer is Not the Same

One of the biggest advances in lung cancer treatment has been the discovery that lung cancer is not a single biological disease. Two patients may both have Stage IV adenocarcinoma but have completely different molecular abnormalities – and therefore require very different treatments.

Modern molecular testing may identify abnormalities involving genes such as EGFR, ALK, ROS1, KRAS, BRAF, MET, RET, HER2 and NTRK. Testing for PD-L1 and, when appropriate, broader next-generation sequencing can further help guide treatment.

If an actionable molecular alteration is identified, a patient may receive a medicine specifically designed to block that pathway. This concept is known as precision oncology.

Can Stage IV Lung Cancer be Treated?

Yes. Although widespread metastatic lung cancer is generally not treated with surgery as the primary treatment, the outlook for many patients has changed significantly.

Targeted Therapy

For cancers containing actionable molecular alterations such as EGFR, ALK, ROS1, RET, MET and others, targeted medicines can produce substantial and sometimes prolonged disease control.

Immunotherapy

Checkpoint inhibitors can help the immune system recognise and attack cancer cells and have produced durable disease control in selected patients.

Chemotherapy

Chemotherapy remains an important part of treatment and is frequently combined with immunotherapy.

Precision Radiation

Radiation can control symptoms and specific sites of disease and, in carefully selected patients with limited metastatic disease, may form part of an aggressive multimodality strategy.

Stage IV does not mean that there are no meaningful treatment options. For some molecularly defined lung cancers, modern treatment can achieve prolonged disease control while maintaining good quality of life.

Why Multidisciplinary Treatment Matters

Successful lung cancer treatment often requires expertise from multiple specialities. At KD Cancer Centre, lung cancer treatment planning may involve Surgical/Thoracic Oncology, Medical Oncology, Radiation Oncology, Interventional Pulmonology, Radiology, Nuclear Medicine, Pathology, Molecular Diagnostics, Physiotherapy and Rehabilitation, Nutrition and Supportive Care.

A multidisciplinary approach helps determine whether the patient requires surgery first, systemic treatment before surgery, chemoradiation, targeted therapy, immunotherapy or another combination.

The objective is simple: The right treatment, in the right sequence, for the right patient.

Frequently asked Questions about Lung Cancer

1. Is Lung Cancer Curable?

Yes, particularly when detected early. Early-stage NSCLC can often be treated with curative intent using surgery. Some patients who cannot undergo surgery may be treated with highly focused radiation such as SBRT. Selected patients with more advanced but still potentially curable disease may require a combination of chemotherapy, immunotherapy, radiation and surgery.

2. Can Someone Who has Never Smoked Develop Lung Cancer?

Yes. Although smoking remains the major risk factor, lung cancer also occurs in people who have never smoked. Some non-smoking patients develop adenocarcinomas containing actionable molecular alterations. Persistent symptoms or a suspicious lung nodule should therefore be investigated irrespective of smoking history.

3. Who should Undergo Lung Cancer Screening?

International guidelines recommend annual low-dose CT screening for selected people at high risk, particularly based on age and cumulative smoking exposure. For example, the American Cancer Society recommends annual LDCT for people aged 50-80 years who currently smoke or previously smoked and have at least a 20 pack-year smoking history. Screening should be individualised, particularly in the Indian population.

4. Is Robotic Surgery Better than Open Surgery for Lung Cancer?

Robotic surgery is a minimally invasive approach rather than a different cancer operation. For appropriately selected patients, it allows lung resection and lymph-node dissection through small ports and may reduce postoperative pain and accelerate recovery compared with open thoracotomy. Complete tumour removal, adequate margins, proper lymph-node dissection and patient safety remain the priorities.

5. What is the Difference between Robotic Surgery and Uniportal VATS?

Both are minimally invasive thoracic surgical techniques. Uniportal VATS generally uses a single small intercostal access incision through which a camera and thoracoscopic instruments are introduced. Robotic surgery uses several small ports and provides magnified 3D vision together with articulated robotic instruments controlled by the surgeon. Selection depends on the tumour, anatomy, complexity and surgical expertise.

6. Does Every Lung Cancer Patient Need Chemotherapy?

No. Treatment depends on the type, stage and molecular biology of cancer. Some early-stage cancers may require surgery alone. Others benefit from chemotherapy before or after surgery. Some patients receive immunotherapy, targeted therapy, radiation or combinations of these treatments.

7. Should Molecular Testing be Performed in Lung Cancer?

Molecular testing is particularly important in non-squamous NSCLC, especially advanced disease, and may also influence treatment in selected earlier-stage cancers. Testing can identify actionable alterations such as EGFR, ALK, ROS1, BRAF, MET, RET, KRAS, HER2 and NTRK. Finding one of these alterations can fundamentally change the treatment plan.

8. What should I do if a CT Scan Shows a Lung Nodule?

Do not panic – but do not ignore it. Most lung nodules are not necessarily cancer. Their significance depends on size, appearance, whether they are solid or sub solid, growth over time, age, smoking and other risk factors, and previous cancer history. Some require CT surveillance, while others need PET-CT, biopsy, bronchoscopy or surgical evaluation.

The Take-Home Message

Detect it early.

Stage it accurately.

Understand its molecular biology.

Discuss it in a multidisciplinary team.

Use minimally invasive surgery when oncologically appropriate.

Personalise systemic treatment according to tumour biology.

Early-stage lung cancer can often be treated with curative intent. For patients requiring surgery, modern techniques including Robotic-Assisted Thoracic Surgery and Uniportal VATS allow many operations to be performed with substantially less surgical access trauma than conventional thoracotomy.

Even when lung cancer is advanced, molecularly targeted treatment, immunotherapy, chemotherapy and precision radiation have created treatment possibilities that did not exist a decade ago.

At KD Cancer Centre, Ahmedabad, our objective is to bring these disciplines together and develop an individualised treatment pathway based on the patient’s cancer stage, tumour biology, lung function and overall health.

Concerned About a Lung Nodule or Lung Cancer?

If a CT scan has detected a suspicious lung nodule, you have persistent respiratory symptoms, or you have recently been diagnosed with lung cancer, a comprehensive evaluation can help establish the diagnosis, stage and appropriate treatment pathway.

Early evaluation can make a significant difference.

Patient information disclaimer: This information is intended for patient education and does not replace individual medical consultation. Screening and treatment recommendations should be tailored to each patient by the multidisciplinary team responsible for their care.

Evidence Base – Selected References

1.
