While pink ribbons for breast cancer have found their way into our conversations, uterine cancer, one of the most common gynaecological cancers affecting Indian women, continues to hide in plain sight. This June, it is time to change that.

The Numbers We Cannot Ignore

India’s cancer burden is growing at an alarming pace. According to national data, India recorded approximately 14.6 lakh new cancer cases in 2022, with projections suggesting this number could rise to over 22 lakh by 2040. Gynaecological cancers are a significant part of this crisis. As per GLOBOCAN 2022 estimates, India reported around 7 lakh new cancer cases in women, and nearly 2 lakh of these were gynaecological cancers. Among the top five cancers affecting Indian women, three originate from the female genital organs, the cervix, ovary and uterus.

Uterine cancer is seeing a steady rise in incidence, particularly in urban India. Research from tertiary care centres indicates that more than 80% of diagnosed patients are from urban areas, highlighting the impact of lifestyle changes on the increasing incidence of this disease. The median age of diagnosis in India is around 54–57 years, meaning most women are diagnosed after menopause: A period when awareness and vigilance are especially important.

Understanding the Disease: Let’s Talk about Endometrial Cancer

When we speak about uterine cancer, we are most frequently referring to endometrial cancer. The uterus is composed of different layers, and endometrial cancer begins in the endometrium, the inner lining of the womb that thickens and sheds during a woman’s menstrual cycle.

It is important to distinguish this from cervical cancer, which affects the neck of the womb, and uterine sarcomas, which arise in the muscle layer of the uterus. Endometrial cancer is the subtype driving the rising numbers we are seeing today and is closely linked to metabolic and hormonal influences.

Why Indian Women are at Growing Risk

For decades, uterine cancer was considered a “western disease” associated with high obesity rates in developed countries. That assumption no longer holds. India’s rapid urbanisation, dietary changes, rising rates of obesity, type 2 diabetes and sedentary lifestyles have created the conditions in which endometrial cancer thrives.

Obesity is the single most important modifiable risk factor. Excess fat tissue produces higher levels of oestrogen, and prolonged exposure to unopposed oestrogen can lead to abnormal, precancerous growth in the uterine lining, women who have been overweight or obese for ten years or longer face a significantly higher risk. With India now home to one of the world’s largest populations affected by obesity, this connection deserves serious attention.

Other Significant Risk Factors Include:

Diabetes and metabolic syndrome, which are increasingly common among Indian adults

Polyendocrine metabolic ovarian syndrome (PMOS) affects millions of Indian women and is linked to chronic hormonal imbalance

Nulliparity (never having been pregnant)

Late menopause

Hormone Replacement Therapy using oestrogen alone without progesterone

A family history of uterine, ovarian or colorectal cancer, particularly Lynch syndrome

Early menarche (first menstrual period before the age of 12)

What is particularly concerning is that many of these risk factors, PMOS, obesity and diabetes, are becoming increasingly common among Indian women. Yet, the connection with endometrial cancer is rarely discussed.

Know the Signs: Don’t Dismiss the Warnings

One advantage of endometrial cancer is that it often presents with symptoms at an early stage. Unlike some cancers that can grow silently for years without obvious symptoms, the most common warning sign of endometrial cancer is abnormal vaginal bleeding, particularly any bleeding, spotting or staining after menopause.

Postmenopausal bleeding is never normal and should never be dismissed as a hormonal fluctuation, stress-related irregularity or an inevitable part of ageing.

Other warning signs include:

Unusual watery or blood-stained vaginal discharge

Pelvic pain or a sensation of pressure

Pain during intercourse

Unexplained weight loss

In India, cultural taboos surrounding reproductive health often lead women to delay seeking medical attention. Many normalise abnormal bleeding or feel uncomfortable discussing symptoms, even with a healthcare professional. Families frequently dismiss these concerns as routine “women’s issues”. This silence can delay diagnosis and treatment. Studies suggest that many women attribute symptoms of uterine cancer to normal ageing, postponing evaluation for months or even years.

The Good News: Highly Treatable When Detected Early

The most important message every woman should hear is that early-stage endometrial cancer is highly treatable and is associated with excellent outcomes. Because abnormal bleeding often prompts medical evaluation, many cases are diagnosed before the disease has spread beyond the uterus. Early detection significantly improves the chances of successful treatment and long-term survival.

A Paradigm Shift in Surgery: Moving away from Routine Lymphadenectomy

Treatment typically begins with surgery. Historically, standard surgical staging for uterine cancer often included a complete lymphadenectomy, the removal of pelvic and para-aortic lymph nodes to determine whether the cancer had spread. While comprehensive, this approach could result in long-term complications, particularly lymphoedema, a chronic and sometimes debilitating swelling of the legs caused by impaired lymphatic drainage.

The Modern Surgical Approach

Today, gynaecological oncology has evolved significantly. For many women with low-risk, early-stage endometrial cancer, routine extensive lymph node removal is no longer necessary.

Instead, advanced techniques such as sentinel lymph node (SLN) mapping, often performed using robotic-assisted surgery, allow surgeons to identify and remove only the first lymph nodes most likely to receive cancer cells. This approach provides accurate staging while minimising surgical trauma and reducing the risk of complications associated with full lymph node dissection.

The Indian healthcare gap

Despite the increasing burden of uterine cancer, access to specialised gynaecological oncology services remains limited in many parts of India. Specialist care is concentrated largely in metropolitan centres, while women in smaller towns and rural communities often face challenges such as long travel distances, financial barriers and limited awareness.

Unlike cervical cancer, which benefits from organised screening initiatives and HPV vaccination programmes, uterine cancer currently has no population-wide screening test. There is no equivalent of a Pap smear for endometrial cancer.

Diagnosis relies on clinical evaluation and endometrial biopsy when symptoms occur.

It makes public awareness not just helpful, but an absolute necessity.

What You can do this June

If you are a woman above 40, pay attention to your body. Any postmenopausal bleeding or unexplained abnormal bleeding should be treated as a warning sign and evaluated promptly by a gynaecologist or gynaecological oncologist.

Managing modifiable risk factors can also make a meaningful difference. Maintaining a healthy weight through balanced nutrition and regular physical activity, as well as controlling diabetes, hypertension and PCOS, may help reduce your risk.

Most importantly, talk about it. Share this information with your mother, sister, daughter, friend or colleague. Open conversations can help break the stigma surrounding women’s health and encourage earlier medical consultation.

Breaking the Silence

Uterine cancer is neither rare nor confined to the West. It is rising quietly in Indian homes, workplaces and communities among women who may be too busy, too embarrassed or simply unaware of the warning signs.

Uterine cancer awareness month is not merely another observance on the healthcare calendar. It is an opportunity to begin a long overdue conversation.

To every Indian woman reading this: your reproductive health matters at every stage of life. Your uterus deserves attention long after childbirth and even after your menstrual periods have ended. Listen to your body, recognise the warning signs, and remember that early detection offers an excellent chance of successful treatment and long-term survival.

Uterine cancer awareness month is observed every June, and the awareness colour is peach. Share this message to help raise awareness and encourage early detection among the women you care about.

By Dr Nishtha Tripathi Consultant
